Purpose

A running log of every batch quantity placed on Hold or Reject, with the reason, so defect patterns are visible and repeat causes can be fixed.

SOP 06 — First 20 Bricks Approval (FIRST20_QC_V1)
SOP 07 — Hourly Production Check (HOURLY_PROD_CHECK_V1)

Defect Reason values

Corner break · Crack · Poor compaction · Wrong size · Interlock mismatch
Too wet mix · Too dry mix · Surface powdering · Colour inconsistency · Handling damage

Rules

One row per hold/reject event; tie it to the Batch ID on the batch card.
Every Hold row needs a follow-up decision (Rework / Scrap / Release) — do not leave it open.
Photograph major defects and reference the photo.
Escalate to Ram/Srinivasan when rejection is unusually high or a full batch is doubtful.
